The purpose of this Brief Plan is to select a health behavior of your own that you want to change and analyze your readiness to change it. Examples of behavior topics you might want to change include exercise, weight management, and sleep, quitting smoking or texting while driving-must be health related.

Do NOT attempt to try to change this health behavior until after you have collected at least 14 days of baseline data. You are required to keep records and self-observations of your selected behavior. Focus on changing one health behavior only.

To prepare for this project, complete the Wellness Inventory and other readings posted on CANVAS.

This is a scholarly written paper so use APA format with the exception of the abstract and table of contents (not required). You may use a combination of narrative format and few “bullet points”. Submit between two to three double-spaced pages (not including title page, contract statement, chart, graph or table of observations), 1-inch margins, and 11 font size. Points will be deducted for grammatical/typographical errors-seek assistance if necessary (e.g., SupportNet) prior to submitting paper. Please be sure to use language/terms covered/discussed in class-this is a requirement. The headings for each section are required and listed as follows:

· Explain why it is important to you.
